CORE offers a Legacy Grant to help fund implementation of a living legacy wall at your hospital. The grant can also fund an innovative hospital-based awareness initiative or event.
The CORE Legacy Grant program encourages organ, tissue and cornea donation awareness and donor designation in the CORE service area of western Pennsylvania, West Virginia and Chemung County, New York, by offering our partnering hospitals financial assistance toward an initiative which supplements CORE’s mission to Save and Heal lives through donation.
- Applicants must be currently employed health care professionals within the CORE donation service area (western Pennsylvania, West Virginia and Chemung County, New York).
- Applicants must submit all application requirements by mail, email or through the form below.
- Acceptance of the CORE Legacy Grant implies that the awardee will meet the required reporting guidelines of the proposed project.

A completed entry form and original Organ Donation Awareness & Donor Designation plan are required. All plans must be typed, double spaced, 1” margins, 12 pt. font in Times New Roman, and contain a title page with title and applicant’s name ONLY. Applicant’s name must be on footer of each page.

Employees of CORE, the grant’s participating sponsors and members of the immediate family of any such persons are not eligible to participant and win. The term “immediate family” includes spouses, siblings, parents, children, grandparents, and grandchildren, whether as “in-laws,” or by current or past marriages(s), remarriage(s), adoption, co-habitation or other family extension, and any other persons residing at the same household even if not related.
