Submit a Quilt Square

CORE invites you to take part in a meaningful tribute honoring those who gave the gift of life through organ, tissue, and cornea donation. Our Remembrance Quilts are made up of personalized squares created by donor families, each one telling a unique story of love, memory, and legacy. Displayed throughout western Pennsylvania and West Virginia, these quilts help inspire others and celebrate the lasting impact of donation.

We encourage you to create a quilt square in honor of your loved one. There is no right or wrong way to design it—your square can be simple or detailed. What matters most is the meaning behind it.

To participate, please print and complete the submission form, then mail both the completed form and your quilt square to:
